Transfer of community-based care services to HCRG Care Group

Community-based care services across Bath and North East Somerset, Swindon and Wiltshire will be provided by HCRG Care Group from Tuesday 1 April 2025.
From 1 April, the group will take on the day-to-day running of these services, while also leading a new partnership, alongside the NHS, local authorities and charities, that will work to transform the way in which community care and support is provided across the region.
Patients are advised that there will be no immediate changes to current services, and care will continue to be provided as before by the same familiar faces, although some colleagues may begin to wear new uniforms and carry updated ID lanyards.
This is an exciting time for community-based care in BSW, as the new partnership will be focussed on delivering better outcomes for the local population, providing greater support for people to live healthier lives, spot the signs of ill health earlier and help those with existing health and care needs live independently for longer.
Shifting the dial towards community-based care is in line with the government’s aims of moving from hospital care to community care, focusing more on prevention digitising the health service.
NHS England recently published its neighbourhood health guidelines, which require a new way of working for the NHS, local government, social care and their partners, so that care and treatment can be joined up and delivered close to people’s homes.
Our new community-based care approach means we have a great foundation on which to build and bring this to life.
Over the last six months, the ICB has worked closely with colleagues from across the local health and care system, including both HCRG and the previous providers, to ensure that the handover takes place as smoothly and as seamlessly as possible, and without any interruption to patient-facing services.
The gradual transformation of community-based care will now begin, with HCRG committed to involving patients, staff and the wider population in any proposed changes, as well as working with the ICB on plans for formal consultation, should a need for such a process arise.
