The Bath and North East Somerset, Swindon and Wiltshire (BSW) Hospital at Home Virtual Wards service enables suitable patients to receive a high level of acute care, assessment, monitoring and treatment from the comfort of their place of residence.

How does Hospital at Home Virtual Wards work?

Your healthcare clinician will asses whether you are suitable to receive care at home from a Virtual Ward team. If you and your carer, if you have one, consent to this, they will refer you into the Virtual Ward. Patients can be referred to the Virtual Ward by the same Health Care Professional who would normally admit a patient into a hospital bed - GPs, paramedics, Consultants and Advanced Nurse Practitioners.

The BSW Hospital at Home Virtual Wards service is lead by Doctors, Medical Consultants and Consultant Nurses/Consultant Practitioners and the team includes
