Our 84 GP practices work with local services as 28 Primary Care Networks (PCNs) across Bath and North East Somerset, Swindon and Wiltshire (BSW). In its simplest terms a primary care network is a group of GP practices working together across a defined area which services a population of between 30,000 and 50,000 people.
PCNs help to make sure that patients are supported and signposted to the health and care professional who is best placed to help – this could be a GP, pharmacist, district nurse, physiotherapist, paramedic, physician associate, as well as staff working in social care or the voluntary sector.
PCNs have been introduced as part of the NHS Long Term Plan to financially sustain general practice in the short term and improve access and care in the longer term.
